If you want to achieve a more youthful appearance with definitive wrinkle improvement, the key lies in the extent of SMAS (Superficial Muscular Aponeurotic System) dissection. Lifting only the skin, or even lifting the SMAS without adequately releasing the retaining ligaments, will lead to early sagging again if only partial elevation is performed.

If only the sagging skin is excised without releasing the retaining ligaments, the lifting effect is limited and short-lived. In addition, excessive skin removal can lead to complications such as wound separation at the incision site, prominent scarring, and deformities like a “pixie ear”.

By carefully preserving the facial nerves and releasing the retaining ligaments, we perform a wide SMAS dissection to achieve a comprehensive lift—improving sagging in the anterior cheeks, nasolabial folds, deep cheek fat, and the jawline.

With sufficient SMAS dissection, only a minimal amount of excess skin is excised—just enough to allow tension-free closure—helping minimize scarring and preserve the natural shape of the ears.
