Hello, I am Dr. Hyo wan Suh, Board-Certified Plastic Surgeon and Representative Director of Lively Plastic Surgery.
When eyelids begin to sag, obstructing your vision or causing persistent skin irritation at the outer corners of the eyes, many consider a corrective procedure known as Upper Blepharoplasty.
As one of the “gold standard” procedures for middle-aged eye rejuvenation, many patients come to my clinic assuming that sagging eyes automatically mean they need an upper blepharoplasty. However, there is one critical factor that is often overlooked:
“Is this procedure truly the best fit for your unique facial structure?”
Upper blepharoplasty is not merely about cutting away excess skin; it is a delicate restoration of both form and function. In some cases, an alternative approach may yield a much more natural and youthful result. At Lively, we prioritize a sophisticated diagnostic process to ensure you don’t just get “a” surgery, but the “right” surgery.

What is the True Goal of Upper Blepharoplasty?
Upper blepharoplasty involves a delicate incision along the planned eyelid crease to tidy up sagging tissue and reinforce the connection between the skin and the levator muscle (the muscle responsible for opening the eye).
Eyelid skin is the thinnest on the body and is constantly in motion. Over time, the loss of elasticity in the muscles and skin can lead to:
- A “sleepy” or tired appearance (Ptosis).
- Functional issues like restricted vision.
- Deep forehead wrinkles caused by overusing forehead muscles to compensate for heavy lids.
The ultimate goal of upper eyelid surgery :
- Functional Restoration: clear the visual axis and resolve skin irritation.
- Aesthetic Vitality: transform a heavy, aged look into one that is bright and spirited.
However, I always emphasize that this is not a “one-size-fits-all” solution. If a patient has significant brow ptosis (droopy eyebrows) or particularly thick eyelid skin, a standard blepharoplasty alone might result in a “stark” or “harsh” look. This is why a precision diagnosis is paramount.

Assessing Your Suitability
The most common question I hear is: “Will I look younger and better after this surgery?”
While the results can be transformative, we must be mindful of the “Golden Ratio.” Upper blepharoplasty naturally narrows the distance between the eyes and the eyebrows. If not planned with precision, this can shift one’s impression significantly.
This procedure is ideal for you if:
- You have moderate to severe eyelid sagging.
- Your eyebrow position is stable (the distance between eyes and brows is sufficient).
- You do not excessively rely on your forehead muscles to open your eyes.
When should you consider an alternative?
- If you already have a beautiful double eyelid but want to remove sagging skin while preserving your natural skin thickness. -> Sub-brow Lift
- If your eyebrows have dropped significantly so the space between your eyes and brows is narrow, or you overuse the forehead muscle to open your eyes -> Endoscopic Forehead Lift
- If you are young, have minimal sagging, and desire a seamless, “born-with-it” crease without a full incision -> Natural Adhesion(non-incisional) Double Eyelid Surgery
